
To define a series of guidelines to be followed to achieve systems for integrating and incorporating ICTs into them.
Divided into work packages – project lasting 36 months.
- 1: Requirements and definition of the practical case (month 2 – month 9).
- 2: Process model services (month 3 – month 9).
- 3: Infrastructure integration and service specification (month 1 – month 12).
- 4: System implementation and testing (month 5 – month 20).
- 5: Preparation of the pilot phase (month 5 – month 22).
- 6: Intervention – first wave piloting (month 15 – month 30).
- 7: Intervention – first wave piloting (month 23 – month 30).
- 8: Evaluation (month 4 – month 30).
- 9: Communication and dissemination (month 1 – month 36).
1. To validate the role that ICT services and applications can play in integrated care (in other words the integration of health, social care and personal care) for any type of life conditions, including topics such as dementia and mobility with health/deterioration.
2. To unblock new services and value chains in active and healthy ageing, including the participation of new actors (such as reimbursement system suppliers, insurers, regional development planners), that lead to the operational implementation of new care pathways and organisational models for integrated care.
- ROLE OF KRONIKGUNE: Together with Osakidetza, Kronikgune coordinates and contributes to the Committed Regions Board by providing the experience of the Basque Country as regards social-healthcare coordination. The Committed Regions Board provides information regarding its current practices to the other partners and committed regions in order to enrich development of the project, which will finally ensure that the results and conclusions of the project are applicable to other European regions.
- ROLE OF OSAKIDETZA: Contributes as a member of the Committed Regions Board by providing its experience and understanding of the Basque Country region to the development of the project and intervention of the pilot studies.
