
General project aim:
The main aim of YOUNG50 is to promote prevention of CVD by transferring the CARDIO 50 screening model, implemented in Italy, to other EU member states. This best practice will be transferred ensuring maximum sustainability and applicability in all sectors of the population, and will be adapted to the target countries to guarantee maximum performance.
Specific project aims:
- Adapt and export the CARDIO 50 screening model to member states participating in the project: Lithuania, Romania and Luxembourg.
- Reduce CVD risk factors by changing unhealthy lifestyles.
- Refine the CARDIO 50 model to obtain maximum performance, sustainability, and dissemination.
Divided into work packages – project lasting 32 months
- 1: Project coordination (Month 1 – Month 36)
- 2: Dissemination and exploitation (Month 1 – Month 36)
- 3: Project evaluation (Month 1 – Month 36)
- 4: Analysis of the situation, viability and needs assessment, and definition of the action plans for the implementation (Month 1 – Month 9)
- 5: Adaptation of procedures and materials of CARDIO50 model and promotion of the YOUNG50 tool (Month 1 – Month 17)
- 6: Implementation of the CARDIO50 model in other countries and preventive intervention (Month 6 – Month 36)
- 7: Follow-up and recommendations (Month 24 – Month 36)
- Exchange of best practice between several EU countries
- Identification, analysis and inclusion in the project of several initiatives, programmes and educational activities and materials.
- Promotion of healthy lifestyles and CVD prevention by means of the creation of networks, associations and cooperation with professional, governmental and non-governmental organisations
- Implementation of the programme in order to increase the chances of adopting a healthy lifestyle, preventing CVD and achieving well-being.
- Knowing more about lifestyle and biological risk factors of CVD in people over fifty participating in the project, and evaluation of the project’s results which will offer guidance to develop future educational and political measures in this area.
- Dissemination of results by means of presentations in scientific meetings and papers in specialised journals.
Kronikgune manages the work package: “Analysis of the situation, viability and needs assessment, and definition of the action plans for the implementation”. They will:
– Describe the current situation in terms of epidemiological context and promotion of cardiovascular health, and prevention activities in the three countries in which the CARDIO 50 best practice will be implemented: Lithuania, Romania and Luxembourg.
– Identify the main features of CARDIO 50 that are essential in order to achieve the desired results in YOUNG50.
– Define the adapted YOUNG50 model in each pilot country, develop the Action Plan and establish the indicators that will enable the programme’s deployment and monitoring. Establish how local prevention programmes will be deployed and supervised.
They will also lead task 6.1 “Preparation for the transfer and implementation of the CARDIO50 best practice” in work package 6 “Implementation of the CARDIO50 model in other countries and preventive intervention”, and task 7.2 “Short-term impact analysis” in work package 7.
