The meeting was held on 23 and 24 October in K\u00f4sice, Slovakia. <\/em><\/p>\n

\"\"<\/a>SCIROCCO Exchange<\/strong> aims to support health and social care authorities to adopt and scale-up integrated care within their healthcare systems. Fourteen partners from nine European regions are the members of this consortium (Germany, Flanders, Basque Country, Slovakia, Slovenia, Vilna, Poland and Puglia), which includes Kronikgune<\/strong> and Osakidetza<\/strong>. Over the 32 months the project lasts, the consortium will work on searching for the assets based on each regions’ evidence to adopt integrated care. Similarly, it will promote personalised knowledge transfer and planning for improvements in integrated care.<\/p>\n

The project’s first general meeting served to share the tasks that have been developed until now. Each pilot presented the results of their self-assessment process for the maturity of integrated care. This part also allowed us to share the experience and opinions of multidisciplinary teams within each region in terms of how the updated \u201cSCIROCCO tool\u201d is used. Kronikgune<\/strong> was responsible for showcasing the experiences and the results of the self-assessment process of the Basque healthcare system.<\/strong><\/p>\n

Additionally, Kronikgune<\/strong> shared its advances related to the work package to identify resources and assets existing in the implementation of integrated care within healthcare systems. This work package has three main goals: identification and mapping of assets, habilitation of its integration within the Knowledge Management Hub,<\/strong> and facilitation of its customised transfer in each region. Kronikgune<\/strong> has designed the strategy defined for the search of assets and literature review, and has agreed with the consortium the next steps to be implemented. Within the upcoming months, two experts from each region are expected to start this search and the first phase of the literature review will come to an end.<\/p>\n

Another matter addressed was the development of the \u201cKnowledge Management Hub<\/strong> – KMH\u201d. This tool will connect existing integrated care assets with the specific needs of the participating regions, while promoting the transfer of knowledge adjusted to said needs.. The functionalities and structure of this tool have already been agreed, and the definition phase of its sustainability strategy is already in progress.<\/p>\n
