The central themes of the congress were people care, the revolution of data and the opportunity to process medical images. Another of the issues tackled was digital transformation as a necessity, but also as an opportunity. The importance of working in the same direction was highlighted, this way we can all advance towards value-based healthcare, which is also efficient and sustainable.<\/p>\n<p style=\"text-align: justify;\">Health professionals, managers, technological companies, universities, researchers and patients gathered once more to share, learn and generate knowledge networks. The ultimate goal is to inspire new projects that are useful for society at large.<\/p>\n<p style=\"text-align: justify;\">At a space reserved for dissemination, <strong>Kronikgune<\/strong> and <strong>Osakidetza<\/strong> had the chance to present the <strong>C3-Cloud<\/strong> and <strong>Scirocco Exchange<\/strong> European projects.<\/p>\n<p style=\"text-align: justify;\"><strong>C3-Cloud<\/strong> (http:\/\/c3-cloud.eu), is a ground-breaking European project that proposes an integrated healthcare model. Its goal is to improve the care of multimorbid patients via personalised healthcare plans. <strong>Osakidetza<\/strong> showcased the two platforms developed during the project:<strong> C3DP<\/strong> <strong>platform<\/strong> integrated within the electronic medical records of the patient and managed by a multidisciplinary team, and the <strong>Patient Empowerment platform<\/strong> that facilitates the participation of the patient and their caregivers, which can be accessed through the<strong> Health Dossier<\/strong>. They also presented the<strong> interoperability of the C3-Cloud and Osakidetza systems<\/strong> addressing the technical (information exchange protocols), structural (translation of forms) and semantic matters (correlation map between concepts and their codes), as well as its privacy and security features. <strong>The Basque Country <\/strong>is one of three pilot regions along with South Warwickshire (United Kingdom) and the region of J\u00e4mtland H\u00e4rjedalen (Sweden). Here, participants will evaluate the acceptability, usefulness, satisfaction, and user-friendliness of the <strong>C3-Cloud System<\/strong>.<\/p>\n<p style=\"text-align: justify;\">Furthermore, <strong>Kronikgune<\/strong> presented the <strong>Scirocco Exchange<\/strong> project and the <strong>Knowledge Management Node (NGC in Spanish)<\/strong> which will be developed with the project. <strong>NGC<\/strong> is a tool that integrates the existing resources to support the development of integrated care (strategies, policies, good practices, guides, etc.). The goal is to help improve the design and implementation of integrated care in the regions, through the design of personalised solutions. It will also facilitate an efficient exchange of knowledge between healthcare systems. Experts in integrated care from each of the regions participating in <strong>Scirocco Exchange<\/strong> will be responsible for identifying the resources.
