The city of Budapest hosted the general assembly on the 14th and 15th of May.\u00a0 <\/em><\/p>\n

\"\"<\/a>The CHRODIS-PLUS<\/strong> Joint Action, which has been operating for 20 months, seeks to address the burden caused by chronic diseases, as well as ensuring sustainability and response from European healthcare systems. 42 beneficiaries from 20 European countries, including Norway, Serbia and Iceland, participate in this initiative. The Basque Country is represented in this Joint Action by Kronikgune <\/strong>and Osakidetza<\/strong>.<\/p>\n

Over the 42 months the project is set to span over, they will implement innovative solutions and promote the transfer of good practices that have proven to be effective in CHRODIS-PLUS <\/strong>to other European regions. 16 policies and 22 pilot projects will make up the core of this Joint Action. The analysis of the policies seeks to raise the awareness of decision makers on how to improve healthcare systems to address chronic diseases.<\/p>\n

The general assembly has been a key event of the project, where over 180 experts in chronic diseases from 26 different countries have debated and commented the real experiences that the project partners have gained during the first 20 months of the initiative. They have addressed the activities developed in the main areas of interest of the project through round tables and presentations. These have focused on the promotion of health and primary prevention, the integrated model for the care of complex chronic patients, the promotion of quality care for people with chronic diseases, empowerment of patients based on ICTs, and employment and chronic diseases.<\/p>\n

Kronikgune<\/strong> was in charge of the design of the common strategy for the implementation of CHRODIS-PLUS<\/strong> actions. To do soy. a guide describing the methods and techniques to facilitate the adoption, implementation and sustainability of the actions undertaken in the pilot regions was drawn up. It was presented at Treviso in a workshop with over 70 consortium members. They also organised several webinars with the partners. The guide is divided into two modules; the first, includes the activities to be developed during the pre-implementation phase, and the second module includes the guidelines for the implementation and post-implementation phases. The Budapest event has served to present the work performed by both institutions in the development of the implementation strategy.
