The event was also used to discuss <strong>personalised plans<\/strong> and the application of <strong>new Information and Communications Technologies (ITCs)<\/strong> to <strong>chronic patients<\/strong>. Many healthcare systems currently focus on offering personalised healthcare plans which include healthcare, social, emotional and family aspects of patients; and which reflect the patient&#8217;s pathway in the system.<\/p>\n<p style=\"text-align: justify;\">The Conference has also featured a space to showcase clinical cases and research projects, round tables as well as practical workshops. Like every year, the Active Patient National Conference, open to citizens, social services and healthcare professionals was held at the same time.<\/p>\n<p style=\"text-align: justify;\">With several European projects, <strong>Kronikgune and the Basque Healthcare Service Osakidetza<\/strong> were present at this annual event for professionals from the public and private sectors on\u00a0<strong>Primary Care<\/strong>,\u00a0<strong>Hospital Care<\/strong>,\u00a0<strong>Mental Health Care<\/strong>\u00a0y\u00a0<strong>Social Care, and research centres. <\/strong><\/p>\n<p style=\"text-align: justify;\">The space dedicated to poster presentation has showcased the European projects <strong>C3-Cloud<\/strong>, <strong>ACT@Scale<\/strong> and <strong>Scirocco<\/strong>. The aim of <strong>C3-Cloud <\/strong>poster <strong>was to <\/strong>unveil the work being undertaken to provide <strong>multimorbid patients<\/strong> with <strong>personalised care plans, <\/strong>supported by ICTs. Thus, work undertaken in the <strong>Basque Country <\/strong>to <strong>launch the intervention<\/strong> and identification of the necessary changes to the <strong>organisational model<\/strong> to implement the<strong> C3-Cloud Care Model<\/strong>, was presented.<\/p>\n<p style=\"text-align: justify;\">The <strong>ACT@Scale<\/strong> poster showed the activities carried out in the <strong>Basque Country<\/strong> for the scaling-up of the integrated healthcare pathway for multimorbid patients. Within the project framework, <strong>12 Integrated Healthcare Organisations (IHOs), the Integrated Care and Chronicity Service (SIAC), the Gorliz Hospital General Management, Santa Marina Hospital and Osakidetza Health Council <\/strong>have redefined the integrated healthcare pathway and the empowerment of these patients and\/or carers.<\/p>\n<p style=\"text-align: justify;\">Finally, the <strong>Scirocco<\/strong> poster featured the maturity analysis of the Basque healthcare system carried out with the <strong>Scirocco model<\/strong>. This European project aims to provide a tool to facilitate scaling-up and transfer of Good Practices in Integrated Care to different European regions.
