The review assessed tasks performed during the first 18 months of the project, analysed any opportunities for improvement and defined specific actions to be carried out in forthcoming months. <\/em><\/h2>\n

\"\"On 8th December, the partners of C3-Cloud<\/strong> Consortium travelled to Brussels to attend the first project review with the European Commission. The aims of the meeting were to analyse the milestones achieved to date, share critical issues and risks identified during work performance and describe the solutions implemented and decisions made to pursue the aims defined in the project.<\/p>\n

The 12 partners of C3-Cloud Consortium,<\/strong> led by the University of Warwick (UK), are currently working to develop a technological architecture seeking to offer on-going, personalised, integrated, multidisciplinary <\/strong>and co-ordinated care<\/strong> to complex chronic patients. To do so, the implementation of two innovative platforms promoting the management of personalised care plans<\/strong> and patient and\/or career empowerment<\/strong> is expected.<\/p>\n

The Basque Country is represented in this Consortium by Kronikgune <\/strong>and Osakidetza<\/strong>. Both organisations play a very active role in the project as they are responsible for two work packages: \u201cDevelopment of application and implementation in pilot regions\u201d and \u201cChange management and development of healthcare pathways and organisational models\u201d. Moreover, the Basque Country<\/strong> is one of the three pilot regions<\/strong> along with South Warwickshire (United Kingdom) and the J\u00e4mtland H\u00e4rjedalen region (Sweden). Both organisations were present at the review and described the work undertaken to date.<\/p>\n

During the first 18 months of the project, the partners carried out different tasks according to schedule. During that period the clinic and technical requirements which will be covered by C3-Cloud architecture<\/strong> have been specified. Likewise, the project interoperability with technologies used by healthcare systems has been guaranteed, while measures aimed at protecting the privacy and confidentiality of patients’ data obtained have been implemented. Another task performed was the analysis of organisational models from the three pilot regions and identification of the conditions needed to deploy the technological architecture. Finally, measurement indicators and instruments for assessment have also been defined.<\/p>\n

The partners are currently involved in completing two platforms and pilot programmes have been launched with the recruitment of centres, professionals and patients and\/or informal carers. These activities are expected to kick off in 2018 and continue for 15 months, with the participation of 1,200 patients across Europe.<\/p>\n
