Its aim was to discuss the experience of European regions and organisations developing and rendering integrated health and social care services.<\/p>\n<p style=\"text-align: justify;\">As part of its engagement in the <strong>European Innovation Partnership on Active and Healthy Ageing<\/strong> (EIP on AHA) and the <strong>Reference Site Collaborative Network<\/strong> (RSCN), Scotland has led the development of a tool to assess regions\u2019 progress and maturity in the provision of integrated care and innovative solutions for active and healthy ageing. The meeting was a good opportunity to explore the potential of this tool called<strong> \u201cScirocco Tool\u201d. <\/strong>The tool is currently being fine-tuned under the framework of the European project <strong>Scirocco. <\/strong>Its use as an instrument to stimulate debate, encourage the transfer of knowledge and experiences, and accelerating the journey of Europe towards integrated care systems, is currently being analysed. Moreover, it intends to facilitate self-assessment of care systems regarding Integrated Care.<\/p>\n<p style=\"text-align: justify;\"><strong>Esteban de Manuel<\/strong>, <strong>Kronikgune<\/strong> Director, acted as a moderator of one of the tables where some partners of the <strong>Scirocco<\/strong> project shared their experiences and lessons learnt regarding the scopes of \u201cPopulation Approach\u201d, \u201dCitizen Empowerment\u201d and \u201cAssessment Methods\u201d.<\/p>\n<p style=\"text-align: justify;\">The Basque health service, <strong>Osakidetza, <\/strong>was also present in the event and took part in the \u201cEuropean Regions &#8216; experience on integrated care\u201d table. <strong>Igor Zabala<\/strong>, Head of <strong>Osakidetza <\/strong><strong>Integrated Chronic Care Service, known as SIAC <\/strong>(Spanish acronym)\u00a0presented the Basque Health System integrated care model.
