The overall goal is to scale-up good practices in <strong>Integrated Care<\/strong> and <strong>Telemonitoring.<\/strong> This intends to contribute to reduce mortality and improve the quality of life of those affected by chronic diseases, frailty or mental illnesses.<\/p>\n<p>The fourth meeting will address the following issues:<\/p>\n<ul>\n<li>reviewing expected project outcomes;<\/li>\n<li>evaluating project progress and management;<\/li>\n<li>closing the learning phase and launching the <em>coaching phase<\/em>;<\/li>\n<li>and planning future activities and transferability actions.<\/li>\n<\/ul>\n<p><strong>Euskadi<\/strong> (Basque Country) is one of the participating regions, along with Catalonia, Groningen, Scotland, the Region of Southern Denmark and the Region of Kinzigtal in Germany. Multi-disciplinary teams made up of health managers and professionals from all the Basque Integrated Health Organisations (IHOs) are working together towards the promotion and scaling-up of two <strong>Osakidetza<\/strong> corporate programmes: the <strong>health telemonitoring service<\/strong> for <strong>patients with heart failure\u00a0<\/strong>and the\u00a0<strong>integrated care pathway for patients with multiple co-morbidities.<\/strong><\/p>\n<p><strong>Kronikgune,<\/strong> representing the Basque Country, was responsible for presenting the progress made in the region to date. Moreover, Kronikgune was also in charge of the assessment and work package related to service selection, and presented the latest results at European level.<\/p>\n<p>As part of the WHINN event, <strong>ACT@Scale<\/strong> organised the work session \u201c<em>Telehealth and Care Coordination: What we did right and what we did wrong? Critical self-reflection<\/em>\u201d, in collaboration with the B3 Action Group of the <strong>European Innovation Partnership on Active and Healthy Ageing<em>\u00a0<\/em><\/strong>(EIP on AHA). The knowledge generated within the project until the event date was shared with the aim of transferring the lessons learnt to other environments.<\/p>\n<p>In that session, Esteban de Manuel, <strong>Kronikgune<\/strong> Director, presented the Basque journey regarding integrated care. Furthermore, <strong>Kronikgune<\/strong> shared its experience implementing a continuous improvement system, Plan-Do-Study-Act (<strong><em>PDSA<\/em><\/strong>).
