Critical aspects identified in the work packages were shared and possible solutions were explored at this meeting.<\/p>\n<p><strong>C3-Cloud,<\/strong> <em>A Federated Collaborative Care &amp; Cure Cloud Architecture for Addressing the Needs of Multi-morbidity and Managing Poly-pharmacy<\/em>, aims to develop a technological and innovative architecture to give <strong>complex chronic patients<\/strong> <strong>ongoing, personalised, integrated, multidisciplinary <\/strong>and <strong>coordinated care<\/strong>. The goals of the project are to create an environment that collects all the information, promote activity coordination and encourage decision-making in the care for patients and\/or caregivers. All this will be achieved by means of the development of two innovative <strong>platforms<\/strong> \u2013 one of them aimed at the development and management of <strong>personalised care plans,<\/strong> supported by <strong>decision-making modules;<\/strong> and the other one focused on active participation and <strong>empowering of the patient and\/or caregiver<\/strong>.<\/p>\n<p>The project lasts 48 months and has 12 European partners, including <strong>Osakidetza<\/strong> (the Basque Health Service) and <strong>Kronikgune<\/strong> representing the Basque Country. <strong>Osakidetza<\/strong>, together with <strong>Kronikgune<\/strong>, is one of the partners in charge of the pilot study that will also be carried out in another two European regions: South Warwickshire in the UK, and the region of J\u00e4mtland H\u00e4rjedalen in Sweden, which have very different healthcare services and facilities. The assessment of the multi-local pilot study over 15 months will enable the creation of a change management guide. This guide aims to facilitate, support and favour implementation in other organisations of a new approach proposed by the project, whatever the organisational and technological features and level of maturity of the interested organisation may be.<\/p>\n<p>Clinical and technical requirements were identified and the <strong>C3-Cloud<\/strong> technological architecture was designed in the project&#8217;s first phase. More ICT (Information and Communications Technology) components were also developed and interoperability aspects were resolved. It is worth noting that the <strong>Basque Country<\/strong>, as well as being a pilot region, is also leading the implementation design, a task in which the <strong>Osakidetza&#8217;s IT Sub-Directorate<\/strong> is actively taking part. The region also presented a report describing the current organisational models of the pilots. It identified changes that are required in the new care model.
