From 14th to 15th March 2017, the European project ACT@Scale<\/strong> consortium met in Groningen (Netherlands). The third general assembly of the project was used to analyse the progress made, the milestones achieved until then as well as future activities.<\/p>\n

\"The<\/a>

The European project ACT@Scale consortium<\/p><\/div>\n

The ACT@Scale<\/strong> project started in March 2016 with the aim of promoting the scaling of good practices in Integrated Care and Telemonitoring. Furthermore, the study will help knowledge transfer regarding the implementation, development and consolidation of good practices to other European regions.<\/p>\n

The scaling of 13 Good Practices is expected, which will facilitate the reduction of mortality and improve the quality of life of people with chronic illnesses, who are frail or have a mental illness. In this way, work to improve some of the Good Practices selected, based on continuous learning and improvement cycles also known as PDSA<\/em><\/strong> (Plan, Do, Study, Act<\/em>) will be carried out. The areas to be worked on are: change management<\/strong>, the selection of services suitable to meet patients’ needs, economic models<\/strong> and system sustainability, and finally, patient<\/strong>\/citizen empowerment<\/strong>.<\/p>\n

The Basque Country participates in ACT@Scale<\/strong> under the leadership of Kronikgune<\/strong> and Osakidetza. <\/strong>At regional level, two examples of good practices which are also Osakidetza<\/strong> corporative programmes have been selected: the telemonitoring service of patients with heart failure and the integrated care path for patients with multiple morbidities. It is worth highlighting that the project receives the collaboration and input of all Osakidetza’s IHOs <\/strong>(Integrated Healthcare Organisations)<\/em>, as well as the SIAC<\/strong> (Integrated Chronic Care Service<\/em>), Osarean and the Health Council. During a first stage, the multidisciplinary work teams who will be working in each of the foregoing areas were put together.<\/p>\n
