{"id":6234,"date":"2016-06-15T16:30:56","date_gmt":"2016-06-15T15:30:56","guid":{"rendered":"http:\/\/www.bio-sistemak.eus\/en\/?p=6234"},"modified":"2016-06-27T11:47:54","modified_gmt":"2016-06-27T10:47:54","slug":"kronikgune-16th-international-conference-on-integrated-care-barcelona","status":"publish","type":"post","link":"https:\/\/www.bio-sistemak.eus\/en\/kronikgune-16th-international-conference-on-integrated-care-barcelona\/","title":{"rendered":"Kronikgune was present at the 16th International Conference on Integrated Care in Barcelona"},"content":{"rendered":"<p style=\"text-align: justify;\"><em>From 23rd to 25th May 2016, Barcelona hosted the <strong>16th Integrated Conference on Integrated Care, <\/strong>where European projects in which <strong>Kronikgune<\/strong> takes part had an outstanding presence.\u00a0 <\/em><\/p>\n<p style=\"text-align: justify;\"><a href=\"http:\/\/www.bio-sistemak.eus\/kronikgune-presente-xvi-congreso-internacional-atencion-integrada-barcelona\/icic16-barcelona-intenational-conference-integrate-care-kronikgune\/\" rel=\"attachment wp-att-6207\"><img decoding=\"async\" class=\"wp-image-6207 alignleft\" src=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2016\/06\/ICIC16-Barcelona-intenational-conference-integrate-care-kronikgune.jpg\" alt=\"ICIC16-Barcelona-intenational-conference-integrate-care-kronikgune\" width=\"453\" height=\"322\" \/><\/a>This annual event aimed at healthcare and socio-healthcare professionals in the public and private sectors was organised by <strong>PIAISS of the Generalitat de Catalunya<\/strong>,\u00a0<strong>Hospital Cl\u00ednic de Barcelona (HCB)<\/strong>,\u00a0<strong>Forum ITESSS<\/strong>\u00a0and <strong>Fundaci\u00f3n TicSalut<\/strong>. The slogan was \u201cA Movement for Change: Enabling People-Centred and Integrated Health and Social Care\u201d. During three days the following matters were discussed: <strong>integration<\/strong> across the care continuum; tackling the challenge of\u00a0<strong>Mental Health<\/strong>; advancing <strong>integrated care;<\/strong> strategic use of <strong>Mobile<\/strong> and <strong>Digital Health and Care<\/strong> Solution and<strong> Good Practices<\/strong> in these fields among others<strong>. <\/strong><\/p>\n<p style=\"text-align: justify;\">As a result of the matters discussed, the <strong>European projects <\/strong>in which <strong>Kronikgune<\/strong> is involved along with <strong>Osakidetza<\/strong> were part of different round tables, speeches, workshops or posters displayed in the exhibition area.<\/p>\n<p style=\"text-align: justify;\">The European project <strong>Carewell <\/strong>was presented in a speech called \u201cUp-scaling of an Integrated Care Model for frail elderly patients\u201d. This project led by <strong>Kronikgune<\/strong> and where <strong>Osakidetza<\/strong> has played an active role, focuses on the development, validation and implementation of a <strong>new integrated healthcare model <\/strong>for complex chronic (multi-pathological) patients. Moreover, the <strong>assessment <\/strong>model to be implemented in the project and the <strong>predictive modelling<\/strong> method currently in use were also presented.<\/p>\n<p style=\"text-align: justify;\">The \u201cTackling the Challenge of Mental Health\u201d section also presented the <strong>MasterMind<\/strong> project where an <strong>on-line cognitive-behavioural therapy <\/strong>designed to <strong>offer people suffering from mild or moderate depression <\/strong>has been developed as a tool to assist in learning new strategies to address depression. The <strong>on-line therapy <\/strong>whose aim is to modify behaviour, thoughts and ultimately the state of mind of these patients, has been deployed in different healthcare organisations in the Basque Country<strong>.\u00a0<\/strong><\/p>\n<p style=\"text-align: justify;\">Furthermore, two European projects <strong>ASSEHS<\/strong> and <strong>Scirocco<\/strong> were presented in a workshop and one round table, respectively. The first event about \u201c<em><strong>Risk stratification <\/strong>tools for Integrated Care: appraisal, deployment and impact in Europe\u201d, <\/em>included the participation of different members of the project consortium and the experience of European regions which did not take part in the research and where risk stratification is used. The recently- started <strong>Scirocco<\/strong> project round table analysed the\u00a0<strong>implementation<\/strong>\u00a0of <strong>good practices <\/strong>identified in the Basque healthcare system.<\/p>\n<p style=\"text-align: justify;\"><em>It<\/em> is worth mentioning the good welcome given to the presentation of the European project <a href=\"http:\/\/www.bio-sistemak.eus\/grupos-inves\/comision-europea\/proyecto-investigacion-united4health\/\"><strong>United4Health,<\/strong><\/a><strong> a research study facilitating the deployment of innovative telemedicine solutions within Integrated Care for Heart Failure patients in Osakidetza. <\/strong><\/p>\n<p style=\"text-align: justify;\">Finally, the \u201c<a href=\"http:\/\/www.bidasoatikmundura.eus\" target=\"_blank\"><strong>Bidasoatik Mundura<\/strong><\/a>\u201d programme for the <strong>promotion of a healthy lifestyle <\/strong>aimed at year 1 and 2 students of compulsory primary education (year 8 and 9 equivalent) carried out as part of the European project Start was also presented.
