{"id":6177,"date":"2016-05-16T09:59:01","date_gmt":"2016-05-16T08:59:01","guid":{"rendered":"http:\/\/www.bio-sistemak.eus\/?p=6177"},"modified":"2016-05-16T09:59:01","modified_gmt":"2016-05-16T08:59:01","slug":"kronikgune-starts-with-a-new-european-project-actscale","status":"publish","type":"post","link":"https:\/\/www.bio-sistemak.eus\/en\/kronikgune-starts-with-a-new-european-project-actscale\/","title":{"rendered":"Kronikgune starts with a new European project: ACT@Scale"},"content":{"rendered":"<p style=\"text-align: justify;\"><em>On 14 and 15 March the Kick-off Meeting for the project in which Osakidetza and Kronikgune are participating took place in Luxembourg.<\/em><\/p>\n<p style=\"text-align: justify;\"><a href=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2016\/03\/proyecto-europeo-act-scale-kronikgune.png\"><img decoding=\"async\" class=\"aligncenter  wp-image-6068\" src=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2016\/03\/proyecto-europeo-act-scale-kronikgune.png\" alt=\"proyecto-europeo-act-scale-kronikgune\" width=\"523\" height=\"294\" srcset=\"https:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2016\/03\/proyecto-europeo-act-scale-kronikgune-300x169.png 300w, https:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2016\/03\/proyecto-europeo-act-scale-kronikgune-1024x577.png 1024w, https:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2016\/03\/proyecto-europeo-act-scale-kronikgune.png 1500w\" sizes=\"(max-width: 523px) 100vw, 523px\" \/><\/a><\/p>\n<p style=\"text-align: justify;\"><strong>ACT@Scale<\/strong>, Advanced Care Coordination (CC) &amp; Telehealth (TH) Deployment at Scale, is a European project that aims to implement, develop and consolidate <strong>best practices <\/strong>in <strong>integrated care<\/strong> and <strong>telemonitoring. <\/strong>The project intends to scale up and transfer learning and knowledge to other European regions. <strong>Health services from five regions <\/strong>are taking part in<strong> ACT@Scale:<\/strong> Catalonia, Southern Denmark, Northern Ireland, North Holland and the Basque Country.<\/p>\n<p style=\"text-align: justify;\"><strong>ACT@Scale<\/strong> aims to <strong>scale up 13 CC&amp;TH programmes<\/strong> in order to have a real <strong>impact<\/strong> on people with chronic diseases, frail patients, the elderly and people with psychiatric problems, and to reduce mortality and improve their quality of life. <strong>Osakidetza<\/strong>, the Basque Health Service, is taking part with two <strong>best practices<\/strong> that will be scaled up: the <strong>telemonitoring service for patients with heart failure <\/strong>and the <strong>integrated prevention programme for patients with multiple comorbidities.<\/strong><\/p>\n<p style=\"text-align: justify;\">The project coordinated by<strong> Philips<\/strong> &#8211; Home Healthcare Solutions works with a consortium made up of <strong>13 partners<\/strong> from several EU countries. <strong>Kronikgune and Osakidetza<\/strong> are leading two work packages: \u201c<strong>Evaluation<\/strong>\u201d and \u201c<strong>Selection and dynamic adaptation of the service<\/strong>\u201d, respectively.<\/p>\n<p style=\"text-align: justify;\">The Grant Agreement was signed so the proyect has been started. <strong>ACT@Scale<\/strong>, the seventh European project that is currently being developed at Kronikgune.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>On 14 and 15 March the Kick-off Meeting for the project in which Osakidetza and Kronikgune are participating took place in Luxembourg.
