{"id":5466,"date":"2015-12-01T13:03:25","date_gmt":"2015-12-01T13:03:25","guid":{"rendered":"http:\/\/www.bio-sistemak.eus\/?p=5466"},"modified":"2015-12-03T09:20:47","modified_gmt":"2015-12-03T09:20:47","slug":"the-european-carewell-project-new-integrated-care-pathways-for-the-multiply-co-morbid-patient","status":"publish","type":"post","link":"https:\/\/www.bio-sistemak.eus\/en\/the-european-carewell-project-new-integrated-care-pathways-for-the-multiply-co-morbid-patient\/","title":{"rendered":"The European Carewell Project: new integrated care pathways for the multiply co-morbid patient"},"content":{"rendered":"<p><!--:es--><\/p>\n<p>The <strong>Basque Country<\/strong>, represented by <strong>Kronikgune <\/strong>and <strong>Osakidetza<\/strong>, leads the European <strong>Carewell<\/strong> project, which mainly focuses on<strong> improving the care and care services provided to multiply co-morbid patients <\/strong>via two pathways: the coordination of<strong> integrated care<\/strong>, and <strong>empowerment<\/strong>, care and support at home by way of the development and use of <strong>new information and communication systems (<\/strong>ICTs).<\/p>\n<p style=\"text-align: center;\"><a href=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/01\/carewell-tic-telemonitorizacion-pluripatologia-osakidetza-kronikgune.png\"><img decoding=\"async\" class=\"size-full wp-image-3509 aligncenter\" title=\"carewell-tic-telemonitorizacion-pluripatologia-osakidetza-kronikgune\" src=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/01\/carewell-tic-telemonitorizacion-pluripatologia-osakidetza-kronikgune.png\" alt=\"\" width=\"462\" height=\"226\" srcset=\"https:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/01\/carewell-tic-telemonitorizacion-pluripatologia-osakidetza-kronikgune-300x146.png 300w, https:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/01\/carewell-tic-telemonitorizacion-pluripatologia-osakidetza-kronikgune.png 770w\" sizes=\"(max-width: 462px) 100vw, 462px\" \/><\/a><\/p>\n<p>The <strong>aim of Carewell<\/strong> is to propose, implement and validate<strong> new integrated care models <\/strong>for<strong> patients with multiple comorbidities <\/strong>that are cost-effective, using different pathways, such as improving <strong>home-based patient care<\/strong>, thereby preventing their hospitalisation, and improving <strong>communication channels between (healthcare and social) professionals and patients and\/or caregivers<\/strong> to facilitate the exchange of information for each patient, thereby avoiding duplication of effort. This will allow patients or their caregivers to communicate with the doctor or advice centre using methods that do not require the patient to attend the health centre.<\/p>\n<p>The importance of <strong>empowerment of the patient and\/or caregiver<\/strong> within this project should be highlighted as they will be educated in their disease and in how to recognise the symptoms. This, together with the use of telemonitoring systems and advice centres, will allow medical staff to perform a better follow-up of the patient&#8217;s evolution.<\/p>\n<p>The new attention and care pathways for the <strong>chronic and multiply co-morbid patient<\/strong> will be piloted at a European level in eight countries. Thus, a 12-month pilot study will allow <strong>new integrated care pathways <\/strong>to be specified, developed and rolled-out in the Basque Country, Apulia, Veneto, Wales, north-west Croatia and Lower Silesia (Poland).<\/p>\n<p style=\"text-align: justify;\"><strong style=\"line-height: 1.5;\">This European project forms part of one of the <strong>strategic lines of the<\/strong>\u00a0<a title=\"Departamento de Salud del Gobierno Vasco\" href=\"http:\/\/www.osasun.ejgv.euskadi.net\/r52-ghhome00\/es\/\" target=\"_blank\">Department of Health<\/a><\/strong><span style=\"line-height: 1.5;\">\u00a0and\u00a0<\/span><strong style=\"line-height: 1.5;\"><a title=\"Web oficial de Osakidetza\" href=\"http:\/\/www.osakidetza.euskadi.net\/\" target=\"_blank\">Osakidetza<\/a><\/strong> that has been worked on since 2009 and in which chronicity, ageing and dependency is one of the key priorities. The work conducted has led to a continual and progressive transformation in the<strong> healthcare model <\/strong>in the Basque Country, which has resulted in an improvement in the <strong>care, quality of life and health outcomes <\/strong>for these patients while allowing a greater <strong>sustainability <\/strong>of the <strong>healthcare system<\/strong>. Over the past few years <strong>Osakidetza <\/strong>has incorporated tools such as <strong>interconsultations<\/strong>, <strong>telemonitoring<\/strong>, the <strong>Health Dossier<\/strong>, the <strong>active patient portal <\/strong>and <strong>follow-up calls<\/strong>, which help to achieve better coordination between professionals, more frequent monitoring of the patient&#8217;s health status and an increased participation of patients in the management of their disease.<\/p>\n<p style=\"text-align: justify;\"><span style=\"line-height: 1.5;\"><strong>Call for tender for
