Biosistemak presents its advances in cardiovascular health and literacy at ICIC 2025 in Lisbon<\/h1>\n

The Institute’s two communications focused on the pilots developed together with Osakidetza, within the framework of the European JACARDI project.<\/h4>\n

Biosistemak was present at the 25th edition of the International Conference on Integrated Care (ICIC 2025)<\/a><\/strong><\/span>, held from 14 to 16 May in Lisbon. This congress, one of the most relevant international meetings in health and social management, brought together thousands of professionals, researchers, managers and community representatives from more than 52 countries under the slogan “Synergies between health and care: harnessing integrated care for a sustainable future”<\/strong>.<\/p>\n

\"\"<\/a><\/p>\n

During the event, our institute presented two oral communications in the framework of the European JACARDI project<\/a><\/strong><\/span>, developed together with Osakidetza<\/a><\/strong><\/span>, focusing on pilots that are being implemented in the Basque Country for the prevention and management of cardiovascular diseases and diabetes.<\/p>\n

The first presentation, given by researcher Yhasmine Hamu<\/a><\/strong><\/span>, focused on the piloting of work package 9, dedicated to care pathways. This project consists of the design, implementation and evaluation of a Heart Failure pathway <\/strong>targeting more than 20,000 people in 13 integrated healthcare organisations (IHOs) <\/strong>in the Basque Country. Yhasmine highlighted the importance of a collaborative approach involving healthcare professionals, managers and the community, with special emphasis on shared decision-making to ensure coordination and continuity of care. This pathway is aligned with the Value-Based Healthcare Strategy <\/a><\/strong><\/span>and also incorporates an innovative gender perspective.<\/p>\n

Irati Erreguerena <\/a><\/strong><\/span>presented the health literacy programme <\/strong>aimed at adolescents and young people, developed in work package 6. Based on the World Health Organization (WHO) Ophelia<\/a> <\/strong><\/span>methodology and a collaborative design, this programme seeks to empower youth through inclusive and community-based approaches. Designed together with adolescents, young health professionals and key community stakeholders, the programme, to be launched in 2026, aims to facilitate access to health care, strengthen self-management and promote healthy behaviours, especially among vulnerable groups at risk of exclusion or stigmatisation.<\/p>\n

Participatory process in Erandio<\/h3>\n

In addition to these oral communications, Biosistemak presented a poster on the community participatory process that is being developed in the municipality of Erandio within the European project Health4EUKids<\/a><\/strong><\/span>, thus reinforcing its commitment to health promotion from a local and community perspective.<\/p>\n

In this European initiative, Biosistemak represents the Basque Country as an Affiliated Entity, coordinating together with Osakidetza two key pilots and co-leading, together with Sant\u00e9 publique France<\/a><\/strong><\/span>, the work package on health literacy. This participation reinforces the positioning of the Basque Country in the adoption of European best practices for the prevention and management of chronic
