Biosistemak presents its model of care for complex chronic patients in the Basque Country at the WHO<\/h1>\n

Scientific director Ane Fullaondo shared the Basque experience in an international webinar on integrated care.<\/h4>\n

The Biosistemak Institute has taken the Basque experience in health management to a global forum, organised by the World Health Organisation (WHO) <\/a><\/strong><\/span>– European Region, in collaboration with the Portuguese Ministry of Health <\/a><\/strong><\/span>and the Central Administration of the Health System (ACSS)<\/a><\/strong><\/span>. During her participation in the webinar “A new era for integrated care<\/em>“, scientific director Ane Fullaondo <\/a><\/strong><\/span>presented the key aspects of the model developed in the Basque Country for the care of complex chronic patients.<\/p>\n

\"\"<\/a><\/p>\n

In his speech, Fullaondo highlighted the main components of the Basque model implemented between 2010 and 2024<\/strong>. This is based on fully individualised care plans that adapt to population risk profiles, combined with an integrated clinical pathway that unifies protocols for patients with multiple pathologies. This is supported by a robust digital ecosystem centred on Osakidetza, which connects all levels of care.<\/p>\n

She also shared a panel discussion with leading international experts such as Dan Alton<\/a><\/span>, <\/strong>National Clinical Advisor, NHS England, and Linda Soikkeli<\/a><\/strong><\/span>, Ministerial Advisor, Ministry of Social Affairs and Health, Finland. The seminar was moderated by Melitta Jakab<\/a><\/strong><\/span>, Head of the WHO European Centre for Primary Care Office, and Vanessa Ribeiro<\/a><\/strong><\/span>, Head of Innovation and Planning at ACSS.<\/p>\n

Exchange of health policies and practices<\/h3>\n

The webinar, which launched a virtual exchange platform on key policies and practices for healthcare integration, brought together international experts to discuss four opportunities identified by the WHO:<\/p>\n

    \n
  1. Digital care pathways for complex patients.<\/li>\n
  2. The potential of risk stratification.<\/li>\n
  3. Community-based care to improve coordination at the local level.<\/li>\n
  4. Integration of health and social care through shared health records.<\/li>\n<\/ol>\n

    This event, which is part of the EU-funded IMPULSO project<\/strong>, aims to promote the improvement of population-based care through the management of Local Health Units (LHUs) in Portugal.<\/p>\n

    This participation consolidates Biosistemak’s position as an international benchmark in the design of integrated care models, the practical implementation of digital tools and the training of multidisciplinary teams.<\/p>\n
