Some elements of the Basque Country strategy have been scaled up in health organizations in seven European countries: Italy, Portugal, Croatia, Denmark, Serbia, Czech Republic and Greece.\u00a0<\/em><\/p>\n

\"\"<\/a>The European JADECARE project<\/strong> finished in October 2023, led by Biosistemak and Osakidetza<\/strong>, has been highlighted by the European Commission<\/strong> for its contribution towards a healthier Europe. Based on collaboration between European countries and with the participation of more than 40 European organizations, its main objective has been to address the transformation of healthcare systems to make them more accessible, equitable and sustainable to offer integrated and person-centred healthcare in Europe.<\/p>\n

For three years, JADECARE has worked to support the transfer of four best practices<\/strong> (Euskadi, Catalonia, Denmark and Germany) among 16 EU countries, allowing 21 regions and more than 4 million people to benefit from innovative solutions. The project has been focused in the reorganization of care pathways, change management, digital transformation or the empowerment of citizens in areas such as integration, chronic diseases, multimorbidity, frail people and patients with complex needs, self-care, prevention and promotion of population health, disease management and case management.<\/p>\n

The good practice of the Basque Country, “Basque healthcare strategy on ageing and chronicity”, transferred to seven countries, gathers the healthcare approaches implemented during the last few years in Osakidetza’s Integrated Healthcare Organizations (IHOs)<\/strong> to face ageing and chronicity, and in search of more accessible, equitable and sustainable care for the benefit of patients and citizens in general. Some of the key elements included in the strategy are population stratification and care plans based on citizens’ needs, continuity of care by improving coordination and communication of services through innovative solutions, and finally patient empowerment and self-management.<\/p>\n

During the three years that the project has lasted, a group of Osakidetza healthcare professionals have collaborated in the different activities developed to transfer knowledge about the digital tools and platforms that have been deployed in Osakidetza and that are for professionals and patients: the unified electronic health record, the \u201cE-rezeta\u201d electronic prescription, the Health Folder, telemedicine for specific patients, the Health Council, Osasun Eskola, the information and training service for citizens to achieve a responsible and active attitude towards their health and disease, or the applications available to citizens.<\/p>\n
