The consortium meeting was held on 14 and 15 June 2023 in Hamburg, Germany. <\/em><\/p>\n

\"\"<\/a>The 6th plenary meeting of the European project ADLIFE<\/strong>, organised by the partner Optimedis, has served to monitor and share the progress of the project, to review the main achievements to date, as well as to share the problems and difficulties encountered, possible solutions and to determine what the next steps will be. During two days, more than 35 people from different organisations and entities participating in the project met. This consortium meeting has also addressed the review of the project that will be carried out with the European Commission next July, in which the project’s development will be evaluated at a technical and economic level between September 2021 and April 2023.<\/p>\n

The European ADLIFE project<\/a>, coordinated at the European level by Biosistemak (formerly Kronikgune),<\/strong> aims to facilitate the personalisation of care plans and improve the capacity for adaptive and early response to patients’ changing needs and preferences through digital solutions. The study targets patients over 55 with severe, chronic disease and a potential reduction in life expectancy due to reduced functionality and capabilities. The project is in the deployment phase of the digital platforms developed in the pilot regions before starting the interventions by training healthcare professionals and recruiting patients.<\/p>\n

In the Basque Country, Biosistemak (formerly Kronikgune) together with Osakidetza<\/strong> are working on the preparation of the Basque pilot. The deployment of the platforms and their integration and interoperability with Osakidetza’s corporate patient data collection systems are in the final phase. Work is currently starting with Osakidetza to involve as many healthcare professionals as possible, the development of training materials and documentation to inform and train both patients and healthcare professionals.<\/p>\n
