The meeting was held on the 28th and 29th of November 2022 in the assembly hall of the Ministry of Health in Madrid. <\/em>\"\"<\/a>The two-day International Conference on Primary and Community Care<\/strong> (https:\/\/cisapyc.com\/<\/a>) aimed to highlight the value of Primary and Community Care, its impact on health and equity, its organization, and the role of each professional as part of the primary care (PC) team, as well as to make the work carried out more visible. The conference brought together more than 250 health professionals, researchers, managers, and decision-makers.<\/p>\n

During the meeting, the value of PC was discussed “from the perspective of international organizations” through the vision of the World Health Organisation (WHO), the Pan American Health Organisation (PAHO), the OECD, and the EU. In addition, the progress of the Primary and Community Care Action Plan and the Strategic Framework was analyzed, “good practices” in Primary Care were shared within the framework of the National Health System (NHS), and the reforms promoted in the EU regarding this first level of care were debated.<\/p>\n

At the round table “International experiences in the face of the current challenges facing Primary Care”, the Director of Kronikgune<\/strong> presented the JADECARE Joint Action<\/strong> coordinated by Kronikgune <\/strong>at the European level. The presentation focused on presenting the objectives of the project with the aim of strengthening the capacity of health authorities to successfully address important aspects of the transformation of the health system towards digital, integrated, and person-centred care. Likewise, the work being done within the framework of the project for the transfer of four good practices to 21 European healthcare contexts was shared, including the Euskadi Good Practice “Basque strategy on ageing and chronicity: integrated care”. The Basque good practice is based on a population-based model based on preventive interventions, patient empowerment, and personalized medical care, with an emphasis on continuity of care, safety, adherence, and improvement of the patient’s experience.<\/p>\n
