In this Joint Action involving 45 partners from 14 European countries, four best practices are being transferred to 21 European healthcare settings. By transferring these four practices, JADECARE proposes to strengthen the capacity of health authorities to successfully address all important aspects of the transformation of health systems, towards digitally enabled, integrated and person-centered care.<\/p>\n<p style=\"text-align: justify;\">The 10 thematic workshops organized under the project have been developed with the aim of:<\/p>\n<p style=\"text-align: justify;\">1) Give visibility to NAs at local\/regional\/national level and boost communication with stakeholders seeking sustainability.<\/p>\n<p style=\"text-align: justify;\">2) Conduct an intermediate self-assessment of the progress of the Good Practices implementation process, including a discussion on the successes and obstacles encountered in the process.<\/p>\n<p style=\"text-align: justify;\">3) Provide structured feedback to the Joint Action&#8217;s management on the methods chosen to plan and implement the Good Practice on the methods chosen to plan and support implementation.<\/p>\n<p style=\"text-align: justify;\">In addition, the workshops have addressed and discussed among the participants the Core Features (CFs) of the major building blocks of the Best Practices, and how <strong>JADECARE<\/strong>&#8216;s Next Adopters are adopting and implementing them in real environments.<\/p>\n<p style=\"text-align: justify;\">Specifically, the Basque Country is transferring the Best Practice &#8220;Basque Health Strategy on Aging and Chronicity&#8221; to eight early adopters. Three thematic workshops have been organized around the Basque practice, each of them focused and aimed at addressing each of the blocks included in the practice. These workshops were attended by experts from Osakidetza and Kronikgune. The following is a description of each of the workshops organized:<\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #33cccc;\"><strong>Block 1 &#8211; Risk Stratification <\/strong><\/span><\/p>\n<p style=\"text-align: justify;\"><strong>Date:<\/strong> June 20 and 21, 2022 &#8211; Aalborg (Denmark)<\/p>\n<p style=\"text-align: justify;\"><strong>Organizer:<\/strong> North Denmark Region (RND) &#8211; NA<\/p>\n<p style=\"text-align: justify;\">Approximately 15 people attended in person and 15 connected online to the workshop. During two days, the Upcoming Adopters who participated in the workshop shared their experiences in the implementation process, such as obstacles, barriers, facilitators, challenges, and lessons learned. Likewise, group activities were organized where the attendees and Kronikgune experts could discuss the aspects, lessons learned and obstacles shared among the participants. Finally, the participants discussed how to reinforce the suitability and sustainability of the implemented practice once the JADECARE project is finished.<\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #33cccc;\"><strong>Block 2 &#8211; Integrated Care <\/strong><\/span><\/p>\n<p style=\"text-align: justify;\"><strong>Date:<\/strong> from June 30 to July 1, 2022 &#8211; Lucca (Tuscany-Italy)<\/p>\n<p style=\"text-align: justify;\"><strong>Organizer:<\/strong> Regional Health Agency Tuscany (ARS Tuscany) &#8211; NA<\/p>\n<p style=\"text-align: justify;\">Representatives from different EU member states participated in the hybrid workshop, which addressed the state of implementation aimed at implementing actions to implement integrated care in the systems. The Early Adopter teams from Italy, Croatia, the Czech Republic, Portugal and the Republic of Serbia presented and discussed their progress, barriers, experiences, challenges, and experience in the implementation process.\u00a0 Likewise, the sustainability actions defined by each of the adopters in the defined action plans to be implemented in the real contexts were exchanged.<\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #33cccc;\"><strong>Block 3 &#8211; Patient empowerment <\/strong><\/span><\/p>\n<p style=\"text-align: justify;\"><strong>Date:<\/strong> June 27-28, 2022 &#8211; Zagreb (Croatia)<\/p>\n<p style=\"text-align: justify;\"><strong>Organizer:<\/strong> Croatian Institute of Public Health (CIPH) &#8211; NA<\/p>\n<p style=\"text-align: justify;\">The workshop was organized in a hybrid format (face-to-face and online) in which 28 participants from different organizations participated. The event was attended by Krunoslav Capak, Director of the Croatian Institute of Public Health in charge of welcoming, Marija Buba\u0161, Director, Croatian Institute of Public Health, Dunja Skoko Poljak, Ministry of Health, Jelena Cura\u0107, the Croatian Health Insurance Fund, Mirjana Kujund\u017ei\u0107 Tiljak, from School of Public Health, Andrija \u0160tampar and the Director of the Medical School of the University of Zagreb who made different presentations. During the workshop, the early adopters presented their experiences, challenges, learning, and the current status of the actions defined in the process of implementing the defined action plans. During the workshop, there was also a discussion on how to strengthen the sustainability of individual JADECARE actions of the partners.
