The 42nd edition of the national congress of the Spanish Society of Family and Community Medicine was held from 30 June to 2 July in Seville. <\/em><\/p>\n

\"\"<\/a>The 42nd edition of the National Congress of the Spanish Society of Family and Community Medicine, Semfyc,<\/strong> (https:\/\/congresodelasemfyc.com\/home<\/a>) held at the Fibes-Palacio de Congresos y Exposiciones in Seville, brought together health professionals from primary care and specialties of the different health services nationwide. For three days, they debated and shared knowledge on scientific and clinical topics such as Covid, medicines, personalized care, chronic diseases, and the approach to palliative patients. Likewise, experiences and good practices were shared on the preventive and community vision of primary care, the digital divide, new health technologies, and new forms of communication and information between professionals and patients.<\/p>\n

The intervention on the management of polypharmacy in complex chronic patients being carried out by Kronikgune together with Osakidetza<\/strong> in the framework of the European Gatekeeper project<\/strong> has been presented through oral communication.\u00a0 The Principal Investigator of Gatekeeper Use Case 7 in the Basque Country, Mar\u00eda Dolores Martines Garc\u00eda, Head of Section, Pharmacy Service of the University Hospital of \u00c1lava-OSI Araba, was in charge of the presentation. The pilot project implemented in 11 Osakidetza Integrated Health Organizations (OSIs)<\/strong> aims to respond to the high prevalence of polypharmacy patients and the problems derived from polypharmacy, such as the inappropriate use of medicines. The main objective is to assess the effectiveness and experience of the intervention using a mobile application (Mi Tratamiento) to improve patient adherence, and to optimize drug prescription with the use of a web platform (CheckTheMeds) integrated into Osakidetza’s systems.<\/p>\n","protected":false},"excerpt":{"rendered":"
