The meeting was held on June 8 and 9, 2022 in Ostersund, Sweden. <\/em><\/p>\n

\"\"<\/a>The 4th plenary meeting of the ADLIFE project<\/strong><\/a> was held in a face-to-face format, which allowed its partners to meet again in situ after the project kick-off meeting held in January 2020 in Bilbao. The meeting brought together 35 researchers and professionals involved in the project with the aim of sharing the progress of the project and reviewing the next activities planned for the coming months.<\/p>\n

The ADLIFE project, coordinated at the European level by Kronikgune<\/strong>, has worked out in the last month in the development of an innovative solution that aims to facilitate the personalization of care plans, and improve the capacity for adaptive and early response to the changing needs and preferences of patients. This European project targeted patients over 55 years of age with severe, chronic disease and a potential reduction in life expectancy due to decreased functionality and capabilities. Through innovative solutions, it aims to improve the quality of life of patients with advanced chronic diseases and provide them with integrated, coordinated, anticipatory, and personalized care.<\/p>\n

Actually, ADLIFE<\/strong>, which started two years ago, is focused to complete the local adaptations of the digital tools, taking into account the messages that best suit each of the health systems where the intervention will be implemented. Likewise, preparations have begun for the deployment of the piloting in the seven pilot regions. It should be noted that Osakidetza<\/strong> is one of the pilot regions where ADLIFE<\/strong> will be tested and is actively participating in the definition of the architecture, the decision support modules, and the patient-centered care strategy supported by digital tools.<\/p>\n

One of the main activities in the coming months at the project level will be the preparation of the piloting, which is scheduled to start in December 2022. Under the project, 846 patients, 679 professionals, and 1183 caregivers are expected to participate. In the pilot in the Basque Country, 126 patients will be recruited and more than 100 health professionals from Osakidetza will participate.<\/p>\n
