During four days, projects, best practices and experiences related to integrated care will be shared from six thematic areas: collaboration with the patient and family, adaptive strategies, knowledge transfer and lessons learned, implementation, innovations and patients and caregivers.<\/p>\n<p><strong>Kronikgune<\/strong> will share its experience in knowledge transfer activities carried out in the framework of the European <strong>Scirocco Exchange<\/strong> project.\u00a0 The workshop that will be organized aims to share the experience of two European regions, Euskadi and Poland in the Knowledge Transfer Program with a particular focus on citizen empowerment and citizen participation in the definition of health system strategies.<\/p>\n<p>The workshop will last 90 minutes, will be divided into two parts. In the first part, in addition to presenting the project in general, the knowledge transfer activities carried out through two pilots in the Basque Country and Poland will be briefly explained. <strong>Kronikgune<\/strong> will be in charge of sharing the knowledge transfer activities carried out with the region of Scotland, in order to learn more about and acquire the knowledge on how to achieve the involvement of patients and end users in the design and delivery of services through the &#8220;double diamond&#8221; methodology.\u00a0 In the second half of the session, an interactive exchange with attendees is planned.
