{"id":6187,"date":"2016-04-12T11:21:05","date_gmt":"2016-04-12T10:21:05","guid":{"rendered":"http:\/\/www.bio-sistemak.eus\/?p=6187"},"modified":"2016-05-16T11:33:17","modified_gmt":"2016-05-16T10:33:17","slug":"prominent-presence-of-kronikgune-european-projects-at-the-8th-national-congress-of-chronic-patient-healthcare","status":"publish","type":"post","link":"http:\/\/www.bio-sistemak.eus\/en\/prominent-presence-of-kronikgune-european-projects-at-the-8th-national-congress-of-chronic-patient-healthcare\/","title":{"rendered":"Prominent presence of Kronikgune European projects at the 8th National Congress of Chronic Patient Healthcare"},"content":{"rendered":"<p style=\"text-align: justify;\"><em>On 7th and 8th April 2016, Madrid (Palacio de Congresos) hosted the <strong>8th National Congress of Chronic Patient Healthcare<\/strong> and the<strong> Third Active Patient National Conference,<\/strong> attended by a large number of professionals in the field.\u00a0 <\/em><\/p>\n<p style=\"text-align: justify;\"><a href=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2016\/04\/congresocro\u0301nicos-kronikgune-proyectos-investigacion.jpg\"><img decoding=\"async\" class=\"  wp-image-6163 alignleft\" src=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2016\/04\/congresocro\u0301nicos-kronikgune-proyectos-investigacion.jpg\" alt=\"congresocro\u0301nicos-kronikgune-proyectos-investigacion\" width=\"334\" height=\"474\" srcset=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2016\/04\/congresocro\u0301nicos-kronikgune-proyectos-investigacion-211x300.jpg 211w, http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2016\/04\/congresocro\u0301nicos-kronikgune-proyectos-investigacion.jpg 650w\" sizes=\"(max-width: 334px) 100vw, 334px\" \/><\/a>This year the motto was \u201c<strong>Making Progress with Results<\/strong>\u201d, with the aim of applying the theory to healthcare practice, making this year&#8217;s congress a meeting point to work on new challenges and define the steps required to address the challenges posed by chronic illnesses in healthcare systems.<\/p>\n<p style=\"text-align: justify;\">New experiences and projects related to the main <strong>chronic strategic lines<\/strong> were presented this year and progress was made in the start-up of care models aimed at improving healthcare focused on chronic patients. In this regard, the <strong>European Projects <\/strong>where <strong>Kronikgune<\/strong> works together with <strong>Osakidetza<\/strong> had a prominent presence in oral communication sessions organised during the event.<\/p>\n<p style=\"text-align: justify;\">Thus, the new healthcare pathway implemented and <strong>Kronik ON<\/strong> empowering programme developed within the <a title=\"Carewell\" href=\"http:\/\/www.bio-sistemak.eus\/research-projects\/european-projects-and-initiatives\/carewell\/\"><strong>Carewell<\/strong><\/a> programme and implemented in different <strong>Osakidetza organisations<\/strong> were presented to the attendees by <strong>Marisa Merino<\/strong>, OSI Tolosaldea Manager and main European project researcher. Furthermore, it is worth mentioning two presentations made on the <a title=\"United4Health\" href=\"http:\/\/www.bio-sistemak.eus\/research-projects\/european-projects-and-initiatives\/united4health\/\"><strong>United4Health<\/strong><\/a> European project, exhibited and presented by <strong>Kronikgune: <\/strong>one from the viewpoint of Osakidetza professionals and the other from healthcare service patients. This study has facilitated the assignment of innovative telemedicine solutions within <strong>Integrated Healthcare<\/strong> for Heart Failure patients within the region.<\/p>\n<p style=\"text-align: justify;\">Likewise, <a title=\"Mastermind\" href=\"http:\/\/www.bio-sistemak.eus\/research-projects\/european-projects-and-initiatives\/mastermind\/\" target=\"_blank\"><strong>MasterMind<\/strong><\/a> and <a title=\"Pre-Start\" href=\"http:\/\/www.bio-sistemak.eus\/research-projects\/european-projects-and-initiatives\/pre-start\/\"><strong>ASSEHS<\/strong><\/a> European projects were also presented in a brief communication session. The first speech focused on the <strong>implementation of Cognitive-Behavioural Therapy (CBT) for patients suffering from depression<\/strong> while the second presented the hurdles and facilitators identified in the <strong>implementation<\/strong> of <strong>Population Risk Stratification <\/strong>in the <strong>Basque Country. <\/strong><\/p>\n<p style=\"text-align: justify;\">Finally, the European study <a title=\"Pre-Start\" href=\"http:\/\/www.bio-sistemak.eus\/research-projects\/european-projects-and-initiatives\/pre-start\/\"><strong>Pre-Start<\/strong><\/a> was presented in poster format on the congress touchscreens.<\/p>\n<p style=\"text-align: justify;\">The <strong>8th National Congress of Chronic Patient Healthcare<\/strong> is an annual meeting point for public and private healthcare professionals in\u00a0<strong>Primary<\/strong>,\u00a0<strong>Hospital<\/strong>,\u00a0<strong>Mental Health<\/strong>\u00a0and\u00a0<strong>Social Healthcare, <\/strong>where attendees had an opportunity to obtain first-hand knowledge of the European research projects where <strong>Kronikgune<\/strong> and <strong>Osakidetza<\/strong> are involved.<\/p>\n<p style=\"text-align:
