The aim of the Conference of Partners is to <strong>generate an open discussion on the achievements<\/strong> so far and to <strong>analyse<\/strong> the future <strong>vision <\/strong>for the <strong>Partnership<\/strong>, in which <strong>innovation and the delivery of tangible results<\/strong> are extremely important.<\/p>\n<p>At a European level, the aim is the <strong>large-scale deployment of innovative solutions<\/strong> for <strong>active and healthy ageing<\/strong>. To do so, the <strong>European Commission<\/strong> has implemented a series of initiatives related to the extension of the EIP-AHA strategy; and the conference will help to discuss and analyse how to maximise the impact of some large scale opportunities. The conference will also serve to recognise the work of different organisations from European regions in relation to the <strong>implementation of innovative solutions<\/strong> for <strong>active and healthy ageing<\/strong>.<\/p>\n<p>In this aspect, <strong>Osakidetza<\/strong> and <strong>Kronikgune<\/strong> have been invited by the organisers to present the European U4H project on 10th December within the &#8220;<em>Working together in partnership to maximise and measure impact<\/em>&#8221; workshop. This project has enabled the<strong> innovative tele-medicine solutions<\/strong> to be deployed in the Basque Country within the<strong> integrated treatment of patients with heart failure<\/strong>.
