It is an opportunity to raise international awareness of the project and receive recognition for it. Countries like Sweden, Lithuania, Latvia, Estonia, Hungary, Israel and the Basque Country through Euskalit take part in this competition. <\/strong><\/p>\n<p>On 9th December, the <strong>Euskalduna Conference Centre and Concert Hall <\/strong>will host the award ceremony for the <strong>&#8220;Quality Innovation of the Year 2015&#8221;<\/strong> Competition in the <strong>Basque Country. Dr. Mar\u00eda Luisa Merino<\/strong>, <strong>Managing Director of OSI Tolosaldea <\/strong>and <strong>head researcher of the project<\/strong>, will collect the award granted to <strong>Carewell <\/strong>for<strong> &#8220;Potential Innovation\u201d <\/strong>of the year in the<strong> Basque Country. Kronikgune <\/strong>as the <strong>project coordinator <\/strong>will also attend the event<strong>. <\/strong><\/p>\n<p>This study aims to <strong>define, implement and validate new integrated healthcare models for complex chronic patients.<\/strong> It is worth highlighting the innovative part of <strong>Carewell, <\/strong>which seeks better care for <strong>patients by implementing new pathways in which coordination and communication of all professionals involved is highlighted, seeking integrated care as well as the use of new methods of patient empowerment<\/strong> and monitoring through face-to-face channels and the use of Information and Communication Technologies ICT.<\/p>\n<p>This is the first year that <strong><span style=\"text-decoration: underline;\">Euskalit<\/span><\/strong> is working and participating in these awards. <strong>23 candidatures<\/strong> were presented to the regional entities and projects selection process for the six categories considered in the awards: <strong>SMEs, large companies, the public sector and non-profit organisations, the social healthcare sector, educational sector and potential innovation.<\/strong> Of all the candidatures presented, <strong>Carewell <\/strong>has been selected by the expert jury made up of the <strong>Board of Euskalit<\/strong> and <strong>Innobasque <\/strong>to represent the Basque Country in the &#8220;<strong>Potential Innovation<\/strong>&#8221; category.<\/p>\n<p>Congratulations to the whole team working on the project and good
