{"id":5526,"date":"2015-12-02T16:39:03","date_gmt":"2015-12-02T16:39:03","guid":{"rendered":"http:\/\/www.bio-sistemak.eus\/?p=5526"},"modified":"2015-12-03T08:59:43","modified_gmt":"2015-12-03T08:59:43","slug":"presentation-of-the-cookbook-for-the-european-act-project","status":"publish","type":"post","link":"http:\/\/www.bio-sistemak.eus\/en\/presentation-of-the-cookbook-for-the-european-act-project\/","title":{"rendered":"Presentation of the Cookbook for the European ACT project."},"content":{"rendered":"<p>Milan will host the <strong><a title=\"EPH Conference Milan\" href=\"http:\/\/ephconference.eu\/\" target=\"_blank\">8th European Conference on Public Health<\/a>, <\/strong>at which the<strong> Cookbook for ACT (Advancing Care Coordination and Telehealth Deployment) will be launched, from 14 to 18 October.<\/strong><\/p>\n<p><a href=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/10\/european-public-health-conference.jpg\"><img decoding=\"async\" class=\"aligncenter size-full wp-image-4575\" src=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/10\/european-public-health-conference.jpg\" alt=\"european-public-health-conference\" width=\"616\" height=\"242\" srcset=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/10\/european-public-health-conference-300x118.jpg 300w, http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/10\/european-public-health-conference.jpg 616w\" sizes=\"(max-width: 616px) 100vw, 616px\" \/><\/a><\/p>\n<p>At 11 Am on 14 October, as part of the session dedicated to \u201c<em>Post-crisis opportunities, from a budget-based approach to achieve better results and performance<\/em>\u201d,\u00a0<strong>Cristina Bescos<\/strong>, <strong>coordinator of the<\/strong><strong>\u00a0<a title=\"ACT\" href=\"http:\/\/www.bio-sistemak.eus\/grupos-inves\/comision-europea\/act\/\" target=\"_blank\">European ACT project<\/a><\/strong>,\u00a0will be responsible for launching and presenting the <strong>project Cookbook, in which both Osakidetza and Kronikgune have participated.\u00a0 <\/strong><\/p>\n<p>This research project, which has recently finished, has identified <strong>best practices<\/strong> in <strong>health system organisational and structural processes <\/strong>in order to support <strong>care integration<\/strong> and the implementation of <strong>telemedicine<\/strong> in a context of the <strong>healthcare provided to chronic patients<\/strong>.<\/p>\n<p><strong>Integrated care<\/strong> is starting to be the path followed by all European countries to overcome the demands of an ageing population and to cope with the needs detected in the provision of care to chronic patients.
