{"id":5448,"date":"2015-12-01T12:29:08","date_gmt":"2015-12-01T12:29:08","guid":{"rendered":"http:\/\/www.bio-sistemak.eus\/?p=5448"},"modified":"2015-12-03T09:25:39","modified_gmt":"2015-12-03T09:25:39","slug":"a-new-award-for-the-european-carewell-project","status":"publish","type":"post","link":"http:\/\/www.bio-sistemak.eus\/en\/a-new-award-for-the-european-carewell-project\/","title":{"rendered":"A new award for the European Carewell project"},"content":{"rendered":"<p>With the slogan \u201c<em>Primary Care: Showing the Way<\/em>\u201d, the <strong>Spanish Society of Primary Care Managers (SEDAP)<\/strong> held its XVII Congress, the objective of which was to share and provide a global vision of the profession and to debate the leadership in the public health model that primary care must take on, in Palma de Mallorca.<\/p>\n<p><a href=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/05\/premio-proyecto-carewell-congreso-sedap.jpg\"><img decoding=\"async\" class=\" wp-image-3779  alignleft\" src=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/05\/premio-proyecto-carewell-congreso-sedap-300x189.jpg\" alt=\"premio-proyecto-carewell-congreso-sedap\" width=\"427\" height=\"269\" srcset=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/05\/premio-proyecto-carewell-congreso-sedap-300x189.jpg 300w, http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/05\/premio-proyecto-carewell-congreso-sedap-320x202.jpg 320w, http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/05\/premio-proyecto-carewell-congreso-sedap-700x441.jpg 700w, http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/05\/premio-proyecto-carewell-congreso-sedap.jpg 960w\" sizes=\"(max-width: 427px) 100vw, 427px\" \/><\/a>The meeting brought together 300 professionals from the sector, and representatives of both <strong>Kronikgune <\/strong>and\u00a0<strong><a title=\"Osakidetza web oficial\" href=\"http:\/\/www.osakidetza.euskadi.eus\/\" target=\"_blank\">Osakidetza<\/a>\u00a0<\/strong>oral presentations concerning two of the European projects in which they are involved, namely, <a title=\"Carewell Proyec\" href=\"http:\/\/www.carewell-project.eu\/home\/\" target=\"_blank\"><strong>Carewell<\/strong><\/a>\u00a0and\u00a0<a title=\"Assehs Proyecto Europeo\" href=\"http:\/\/assehs.eu\/\" target=\"_blank\"><strong>ASSEHS<\/strong><\/a>;\u00a0there was also a poster concerning the <strong>Pre-Start<\/strong> project. The communication about the <strong>European Carewell project, <\/strong>entitled<strong> \u201cImplementation of integrated care for multiply co-morbid patients in the Basque Country\u201c<\/strong>, presented by Dr. Mar\u00eda Luisa Merino, Managing Director of the OSI Tolosaldea, was awarded the best oral communication prize at the conference. A new prize awarded to <strong>Carewell<\/strong>, the objective of which is to define, evaluate, implement and validate new and cost-effective integrated healthcare models for complex chronic patients via different pathways, such as the improved coordination of healthcare professionals, promotion of home-based patient monitoring and promotion of the ability of patients and caregivers to self-manage the disease.<\/p>\n<p>These interventions aim to have a positive impact on care quality and efficiency and the satisfaction of both professionals and patients alike. An educational programme, known as the <strong>Kronik ON Programme\u00a9<\/strong>, which involves sessions to promote disease awareness and control, has been developed in the framework of the project. Educational materials that will be issued to patients\/caregivers and which will also be made available in the\u00a0<strong><a title=\"Osakidetza official web site\" href=\"http:\/\/www.osakidetza.euskadi.eus\/r85-ghhome00\/es\/\" target=\"_blank\">Osakidetza<\/a>\u00a0<\/strong>section of the\u00a0<strong><a title=\"Osasun Eskola Osakidetza web site\" href=\"http:\/\/www.osakidetza.euskadi.eus\/osasuneskola\/es\/\" target=\"_blank\">Osasun Eskola<\/a>.<\/strong>\u00a0web site has also been prepared.
