Thus, <strong>empowerment allows patients to take an active role in managing their disease <\/strong>by acquiring greater control over the decisions and actions that affect their health.<\/p>\n<p><a href=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/07\/kronikgune-kronik-on-programa-formaci\u00f3n-paciente-carewell.jpg\"><img decoding=\"async\" class=\"aligncenter size-full wp-image-5867\" src=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/07\/kronikgune-kronik-on-programa-formaci\u00f3n-paciente-carewell.jpg\" alt=\"kronikgune-kronik-on-programa-formaci\u00f3n-paciente-carewell\" width=\"810\" height=\"215\" srcset=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/07\/kronikgune-kronik-on-programa-formaci\u00f3n-paciente-carewell-300x80.jpg 300w, http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/07\/kronikgune-kronik-on-programa-formaci\u00f3n-paciente-carewell.jpg 810w\" sizes=\"(max-width: 810px) 100vw, 810px\" \/><\/a><\/p>\n<p><strong>Within the framework of the <strong>European <\/strong>\u00a0<a title=\"Proyect Carewell\" href=\"http:\/\/www.bio-sistemak.eus\/en\/research-projects\/european-projects-and-initiatives\/carewell\/\" target=\"_blank\">Carewell<\/a><\/strong>,\u00a0<strong>project<\/strong>, the aim of which is to improve the care of chronic patients by enhancing coordination between healthcare professionals and following-up and empowering patients, a group mainly comprising <strong>nursing staff <\/strong>has developed a structures programme for the empowerment of complex chronic patients\u00a0(multiple co-morbidities), known as the <strong>Kronik ON programme\u00a9. <\/strong>The aim of this programme is to allow patients to understand their diseases, helping them to detect and control their warning signs and symptoms, thus allowing them to develop the capacity to <strong>self-manage their diseases<\/strong>. <strong>An additional aim of the programme is to educate patients in how to adopt the healthy lifestyles recommended for their diseases.<\/strong> The programme also contemplates training activities for the caregivers of these patients to increase their ability to manage the diseases concerned as many of these patients are unable to look after themselves and depend to a large extent on informal caregivers.<\/p>\n<p>The <strong>Kronik ON Programme\u00a9<\/strong> comprises four training sessions covering topics such as evaluation of the patient, explanation of the diseases identified, treatment adherence, healthy lifestyles and self-control of warning signs and symptoms.<\/p>\n<p>During the <strong>empowerment sessions<\/strong>, the reference nurses will use <strong>materials <\/strong>regarding the <strong>most prevalent diseases in these patients<\/strong>, such as educational materials on heart failure, diabetes or COPD. In addition, patients will be provided with information sheets concerning their diseases.
