{"id":5432,"date":"2015-12-01T11:52:13","date_gmt":"2015-12-01T11:52:13","guid":{"rendered":"http:\/\/www.bio-sistemak.eus\/?p=5432"},"modified":"2015-12-03T09:29:18","modified_gmt":"2015-12-03T09:29:18","slug":"the-basque-country-and-its-carewell-project-receive-an-award-from-correo-farmaceutico-as-one-of-the-initiatives-of-the-year","status":"publish","type":"post","link":"http:\/\/www.bio-sistemak.eus\/en\/the-basque-country-and-its-carewell-project-receive-an-award-from-correo-farmaceutico-as-one-of-the-initiatives-of-the-year\/","title":{"rendered":"The Basque Country and its Carewell project receive an award from Correo Farmac\u00e9utico as one of the initiatives of the year"},"content":{"rendered":"<p>The Carewell project led by the Basque Country, via <strong>Osakidetza <\/strong>and <strong>Kronikgune<\/strong>, has been selected as one of the \u201c<strong>Initiatives of the Year<\/strong>\u201d in the Management category by <strong>Correo Farmac\u00e9utico<\/strong>, a publication that recently celebrated its <strong>XII Awards Ceremony<\/strong>, which aims to reward the daily work of people whose contributions help to improve the world of healthcare.<\/p>\n<p><strong><a href=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/05\/proyecto-europeo-carewell-premio-farmaceutico1-1024x551.jpg\"><img decoding=\"async\" class=\" wp-image-4046  aligncenter\" src=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/05\/proyecto-europeo-carewell-premio-farmaceutico1-1024x551-1024x551.jpg\" alt=\"proyecto-europeo-carewell-premio-farmaceutico1-1024x551\" width=\"483\" height=\"260\" srcset=\"http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/05\/proyecto-europeo-carewell-premio-farmaceutico1-1024x551-300x161.jpg 300w, http:\/\/www.bio-sistemak.eus\/wp-content\/uploads\/2015\/05\/proyecto-europeo-carewell-premio-farmaceutico1-1024x551.jpg 1024w\" sizes=\"(max-width: 483px) 100vw, 483px\" \/><\/a><\/strong><\/p>\n<p><strong>Carewell <\/strong>is a European project that studies new <strong>integrated care pathways <\/strong>for multiply co-morbid patients in which Italy, Wales, Poland and Croatia also participate<\/p>\n<p>This project focuses on improving care and care services for <strong>complex chronic patients <\/strong>and social care needs via two pathways, namely <strong>coordination of integrated care<\/strong> and empowerment, care and home-based support by developing and using<strong> new information and communication technologies <\/strong>(ICTs).<\/p>\n<p><strong>Carewell <\/strong>is one of the <strong>strategic lines shared by the Basque Regional Government&#8217;s Department of Health <\/strong>and <strong>Osakidetza<\/strong>, specifically as part of the <strong>proactive action for combatting the challenges <\/strong>of chronicity, ageing and dependency, as well as the sustainability of the health system and the ability to innovate to improve <strong>care<\/strong>, <strong>quality of life <\/strong>and health outcomes for patients.<\/p>\n<p>&nbsp;<\/p>\n<p>Over the past few years <strong>Osakidetza <\/strong>has incorporated tools such as the electronic prescription, <strong>interconsultations<\/strong>, <strong>telemonitoring<\/strong>, the<strong> Health Dossier<\/strong>, the<strong> active patient <\/strong>gateway and <strong>follow-up calls<\/strong>, which are helping to ensure better coordination between professionals and to increase the participation of patients in the management of their disease.<\/p>\n<p>The <strong>Head of Healthcare Research and Innovation, Mar\u00eda Aguirre, <\/strong>and the <strong>Director of Pharmacy<\/strong>, <strong>I\u00f1aki Betolaza<\/strong>, collected the award on behalf of the <strong>Department of Health<\/strong>. Aguirre noted that \u201c<em>this is recognition of the work of numerous professionals who contribute every day to the transformation and improvement of the Basque public health system so that it can continue to offer the very best care to the public<\/em>\u201d.<\/p>\n<p>She also highlighted the importance of <strong>cooperation with European partners <\/strong>and the recognition of the ability of the health system to contribute to developments of global interest, noting that the <strong>Basque Country <\/strong>is<strong> one of the reference regions in the field of active and healthy
