A total of <strong>14 European partners <\/strong>are participating in the project and <strong>evaluation<\/strong> is being performed in <strong>5 regions<\/strong>, namely the <strong>Basque Country, Catalonia, Lombardy, Groningen and West-Lothian, <\/strong>in which different aspects of <strong>health systems<\/strong> are being evaluated <strong>quantitatively and qualitatively<\/strong>. The aspects being evaluated are<strong> health outcomes, population stratification, organisational models <\/strong>and <strong>continual care processes <\/strong>and, finally, <strong>adherence of both professionals and patients<\/strong>.<\/p>\n<p><strong>In the Basque Country, Kronikgune and <\/strong>\u00a0<a title=\"Oficial site of Osakidetza\" href=\"http:\/\/www.osakidetza.euskadi.eus\/\" target=\"_blank\"><strong>Osakidetza<\/strong><\/a>\u00a0are jointly responsible for analysing the <strong>Integrated Intervention Plans (IIPs) <\/strong>for all Integrated Healthcare Organisations (IHOs) in the <strong>Autonomous Community of the Basque Country<\/strong>. <strong>Quantitative information<\/strong>, namely health indicators for 2012 and 2013 for all IHOs, and qualitative information based on questionnaires for both healthcare professionals and patients, is being <strong>compiled<\/strong> to <strong>undertake the analysis<\/strong>, and a focus group is being organised for patients from the <strong>IHO Barrualde \u2013 Galdakao<\/strong>.<\/p>\n<p>The consortium has a further 8 months of work ahead to achieve a consensus on the best <strong>organisational <\/strong>and <strong>structural procedures <\/strong>to support the large-scale <strong>implementation <\/strong>of <strong>CC and TH services<\/strong>, which will be gathered together in a
